TITLE:                               Incident Investigation Specialist 
DEPARTMENT:                 Incident Investigation

 

I. POSITION OBJECTIVE 

Conduct moderate level investigations as well as support Corporate safety programs at the Corporate and site level. Ensure accuracy and completeness of investigations. Serve as an Investigation Team Leader in the KMS Investigator database. Be knowledgeable in KMS Investigator and Tracker module(s). Assist in verifying incident types are correctly identified and finalize investigation reports. Demonstrate analytical and problem-solving abilities. Conduct investigation interviews to obtain evidence needed for the investigation.


II. MAJOR AREAS OF ACCOUNTABILITY

  • Lead and Facilitate Investigations: Work as a problem solver and facilitate level II and level III incidents as requested at the site level. Present finalized investigations to management, as requested.
  • Quality Assurance: Conduct QA on investigations submitted by site team leaders to ensure consistency in report formatting and usage of methodology.
  • KMS Reviewer: Serve as one of the KMS H&S reviewers for review of TX incidents initiated/closed in KMS.
  • Train: Conduct one-on-one training sessions with site facilitators to aid in educating the expanding their knowledge which ultimately improves site investigation quality.
  • Support: Assist site-level team leaders, as needed, to improve competency and ensure completeness of detailed investigations. Provide day-to-day support to Corporate Safety management for incident investigation. Support Corporate Safety training activities. Assist KMS team with incident trending, as needed. Work with 3rd party consultants, as needed to support incident investigation goals. Assist with procedure revisions for Corporate Safety team including application of tools and resources. Assist with TX site quarterly incident investigation networking meetings.
  • THM: Promote cleanliness and order through the implementation of the Total Housekeeping Maintenance program. Emphasize Nothing Touch Ground program and No Visual Pollution program. Maintain the plant in like new condition.



III. QUALIFICATIONS

  • High School diploma or equivalent required with (10) years above-mentioned experience. Bachelor degree [preferred] in engineering disciplines, criminal justice, safety or PSM-related disciplines with at least (6) years of above-mentioned experience
  • At least (6 – 10) years of applicable safety or incident investigation experience depending on education. Knowledge of EHS and PSM procedures. Preferred experience with leading incident investigations utilizing Cause Mapping.
